Support Coordination & Specialist Support Coordination

A real human who speaks fluent NDIS.

One coordinator, every provider on the same page, and a plan that finally moves — with budgets tracked, service agreements sorted and someone loud in your corner.

Why coordination with Accucare feels different

Independent, honest, in your corner.

Support Coordination goes sideways when it's a shared inbox, a rotating cast and a coordinator conflicted about which providers they push. We built ours the opposite way — one named human, transparent budgets, and a duty to recommend the right fit even when it isn't us.

One coordinator. Real relationship.
You get a named person, their direct mobile, and a backup — not a shared inbox.
Independent by design
We coordinate every provider fairly — including recommending providers other than Accucare.
Conflict-of-interest clean
Documented, disclosed and reviewed. You always know what hat we're wearing and when.
Capacity-building, always
We show you how the system works so you can drive more of it yourself over time.
Six things a great coordinator actually does

More than admin. Way more.

The full scope of Support Coordination — the visible bits and the behind-the-scenes graft that makes a plan sing.

Plan translation

We turn your NDIS plan from PDF-speak into a clear week-by-week action list — no jargon left behind.

Provider matchmaking

We shortlist providers who actually fit — vibe, values, availability — not just whoever answers first.

Service agreements

We read every service agreement, flag the weird bits and make sure hourly rates align with your plan.

Advocacy on your side

Providers dropping the ball? Plan under review? Complaint to lodge? We do the loud calls for you.

Budget tracking

Monthly snapshots of what's spent, what's left and what to pull forward — no surprise underspends.

Specialist coordination

Complex situations, hospital discharges, safeguarding, housing — Level 3 coordinators available.

The shape of our coordination

Skilled coordinators. Small caseloads. Real answers.

We cap caseloads on purpose so every participant gets a coordinator with time to actually pick up the phone — not a triage queue.

01

02
Provider stack

Shortlist, interview and sign the right providers. Cheat-sheet of who does what, when and why.

03
Get moving

Services start, budgets tracked live, monthly check-ins to make sure nothing slips.

04
Ready for review

By plan review we've built a paper trail that proves what's working and what you'll ask for next.

Is coordination right for you?

Coordination fits when the plan feels bigger than the bandwidth.

If any of these sound familiar, you probably have (or should ask for) Support Coordination funding in your NDIS plan.

Chat with a coordinator
  • You've got a plan but no idea where to start.
  • You're juggling too many providers and none of them talk to each other.
  • You had a plan review and got funding you don't fully understand.
  • Something urgent has changed — hospital discharge, housing, safeguarding.
  • You want someone independent recommending providers, not selling their own.
  • You want capacity building — to run more of this yourself next plan.
let's untangle the plan

Ready for someone to actually pick up?

Send us your plan, or just the general shape of things. We'll book a free 30-minute chat and tell you honestly whether we're the right coordinators for you.

Good to know

Support Coordination — how it works

Eligibility, referrals, timelines and what happens next — answered.

Do I qualify for Support Coordination?+

If your NDIS plan includes Support Coordination (Level 2) or Specialist Support Coordination (Level 3), yes. We work with both — no referral bottleneck.

How do I get started?+

Send us your plan. We'll book a 45-minute onboarding call within 3 business days and walk you through your budgets, goals and options.

How long until things start moving?+

First provider intros happen inside week one. Full service stack (SIL, therapy, community, transport) is usually live within 4–6 weeks.

What happens next after I sign the Service Agreement?+

1) Plan translation session. 2) Goal mapping. 3) Provider shortlist and intros. 4) Bookings made and budgets tracked. 5) Fortnightly check-ins and plan-review prep.

Is Support Coordination independent of your other services?+

Yes — you can use our coordinators without using our SIL, day programs or in-home care. Conflict of interest policy applies and we tell you upfront.

Can you help prep my plan review?+

It's what we do best. Evidence pack, provider reports, goal progress and a rehearsed narrative — all ready before your review meeting.
